Welsh Ambulance Service Trust
Frequently attending patients are those who attend Emergency Departments or other health care facilities/services more regularly than other members of the population. We work with WAST to understand triggers and crisis points that bring patients to the point of needing help regularly and create bespoke plans that involve all agencies across the board. help reduce the patient need for unscheduled services and increase their health and well-being by ensuring they get consistent, appropriate care from the right provider.
Mid and West wales Fire & Rescue Service - Safe and Well Visits
A Safe and Well Visit includes all the contents of the home fire safety checks but will also include other safety messages that may be relevant to the people living in the property. The five main additional topics will cover:

Optometry
Optometrist Independent Prescribers enable them to manage acute eye problems within the primary care optometric setting. This practitioner will act as a prescribing hub for the cluster, therefore decreasing the numbers of referrals from primary care optometrists to GPs and secondary care therefore freeing up capacity in these settings.
Cluster First Contact Practitioner – Musculoskeletal Specialist
An MSK will see patients attending the surgery with joint/muscle pain. Alternatively a self referral can be made to physiotherapy by clicking Pembrokeshire physio self referral
