New patients are welcomed within our practice. In order to register with the Practice you will be required to complete a Patient Questionnaire and an NHS Registration Form, once completed, you will be provided with a copy of the Practice Leaflet. This practice leaflet will tell you when the surgery is open, clinic details, how to make an appointment for a consultation, the arrangement for getting advice over the phone, criteria for home visits, prescribing policies and procedures, practice staff and additional services the Practice provide.
When you register with the Practice for administration reasons you will be placed on one of the GP partners lists. However, you are allowed to request an appointment with any of the GPs within the Practice. Please inform the receptionists at the time of booking if you have a preference. You may have to wait longer to see a preferred practitioner.
As a Practice, we ensure that patients are not unfairly discriminated against on grounds of age, gender, disability, ethnicity, race, religion or sexual orientation.
NHS Wales is using a new system to provide a faster and more efficient service to patients transferring between practices so we need your NHS number to register you.
Everyone registered with the NHS in England, Wales and the Isle of Man has a unique patient identifier called NHS Number. Most GP practices will send you a letter containing your NHS Number when you register with them.
Your NHS Number helps healthcare staff and service providers identify you correctly and match your details to your health records. This will ensure you receive safe and efficient care within the NHS.
Patient Responsibility
You are responsible for looking after your own health and welfare.
Make every effort to attend any appointments made promptly.
Advise the surgery if you are unable to attend an appointment so that it may be offered to another patient.
Advise of any change of name, address and circumstances.
Be aware of when repeat medication is due and allow sufficient time for the surgery to generate the prescription and for the pharmacy to dispense it.
Help our receptionists by giving them as much information as possible to ensure you are given the relevant appointment with the Doctor, Practice Nurse, Pharmacist or Healthcare Assistant. This information can be given in private, away from the main reception desk if required.
Whenever possible please plan ahead, appointments are available to be booked four weeks in advance.
Please ensure you telephone the surgery after 2 pm for the results of investigations e.g. x-ray, blood etc.
